Accuracy is an all or nothing card. If you are running a shiv deck and you expect to beat the heart at ascension 20, you must normally run at least one Accuracy. The time-eater greatly counters shiv decks and accuracy allows each card to be worth playing. The same goes for the heart. I would never pick accuracy unless the deck's win condition was solely shivs.
